Purpose: To recognize all club secretaries, within our District, who have
completed the requirements listed below. By doing so, these secretaries have
made it easier for the District Governor and the Cabinet Secretary to complete
their required reports.
Eligibility: All District 35-R Club
Secretaries who have served
during the above Lionistic year.
REQUIREMENTS:
1.___All monthly 'MEMBERSHIP" reports have been submitted by the 5th of each
month.
2.___Annual PU-101 Form completed and submitted to the District and LCI by May
1st.
3.___Submitted completed MD and International Convention delegate forms on time.
4.___Submitted annual Activities form in a timely fashion.
5.___Attended all your Zone meetings.*
6.___Attended all 35-R Cabinet meetings.*
7.___Attended a Secretaries training session.
8.___Kept accurate minutes of all club meetings, attendance records and records
as required.

Please mail this form to the District
Governor by July 15th so that the application can be processed and the Award
will be presented at the first Cabinet meeting of the next year.
Note: *One excused absence will be allowed
with prior notification to the Zone Chairman.
